Volunteer Service Form

For possible consideration of the undersigned for a Board/Committee position, a criminal background check may be obtained. By submitting this completed form to the City Clerk, the undersigned authorizes and consents to the City of Enterprise performing a criminal background check on the undersigned and the undersigned consents to the disclosure of the results to city personnel. If such is required, the undersigned will submit his/her social security number and date of birth to the City Clerk for said purposes.

Note: This completed form may be considered a public record subject to inspection/disclosure to the public.
